We are delighted to announce an exciting new project starting in 2020, a universal offer which helps ensure families understand, are signposted to and have access to all available services from early pregnancy through their early childhood.

The ‘Your Family’ project will make and maintain contact, assess the needs and provide support to every family living in the ABSS wards; from early pregnancy until their youngest child reaches four years old.

The project will focus on supporting families with their social and emotional wellbeing, while ensuring all families access our full range of services relating to all our Outcomes.

In partnership with each family, an individualised ‘Your Family Plan’ will be created ensuring families understand, are signposted and have access to all available 0-19 services. It will focus on prevention and early intervention including those commissioned and provided by ABSS, community organisations and statutory services.

It will ensure all families in our wards have the opportunity to access and benefit from our full and comprehensive offer, which will be co-ordinated, inclusive and accessible. It will identify ABSS families with additional needs early in order to offer them appropriate support and intervention (either linking in to our current offers or statutory offers).

A number of Partners will deliver the service, with the ‘Your Family’ team a distinct and discreet multi-disciplinary team hosted by Southend-on-Sea Borough Council. The service will be led by a specialist health visitor, with health visitors, ‘Your Family’ workers and other specialist professionals both inside and outside of the health system.

Your Family will offer a series of additional ‘contact points’ for families, delivered in conjunction with the Healthy Child Programme*, at important times in the families’ journey. This means that in addition to the current statutory Health Child Pathway contact points, families in ABSS areas will receive an additional five contact points (currently planned at 11 weeks; 14-18 weeks; 6 months; 18 months and three to three and a half years) linking them to the full comprehensive ABSS offer.

* The Healthy Child Programme for the early life stages focuses on a universal preventative service, providing families with a programme of screening, immunisation, health and development reviews, supplemented by advice around health, wellbeing and parenting. The programme is led nationally by health visiting and delivered through a partnership approach.
